The 8D method helps teams respond to quality problems systematically. It emphasizes team-based investigation, containment, root-cause verification, corrective action, and prevention of recurrence.

The 8D process

1. D1 — Establish the team: Bring together the right people.
2. D2 — Describe the problem: Define the issue clearly and factually.
3. D3 — Contain the problem: Protect customers while investigating.
4. D4 — Find the root cause: Verify what caused the issue and why it escaped detection.
5. D5 — Select corrective actions: Choose actions that address the verified cause.
6. D6 — Implement and validate: Confirm that the actions work.
7. D7 — Prevent recurrence: Update processes and controls as needed.
8. D8 — Recognize the team: Capture lessons learned and acknowledge contributions.
